Office Financial Policy

Our primary goal is to continue to provide our patients with the finest personalized dental care possible. We also understand that many people cannot afford to pay for their treatment at the time of service. The following payment options will be available to our patients.
  1. Full payment at the time of service. Payment may be made with cash, check, VISA, MasterCard, American Express and Discover.
  2. For extended treatment, such as a crown, fixed bridge work, etc., a three payment plan may be made with the payment in full by completion of treatment.
  3. After researching companies offering health care payment plans, we've selected CareCredit for those patients who need to extend their payments over a longer period of time.
  4. For our patients who are unable to meet any of the above options, we will help arrange other means.


Our office is committed to helping you maximize your insurance benefits. Because insurance policies vary greatly, we can only estimate your coverage in good faith, but cannot guarantee coverage due to the complexities of insurance contracts.

If you carry dental insurance, please be aware that the patient is responsible for any balance that your dental plan does not cover. Your estimated patient portion must be paid at the time of service. As a service to our patients, we will bill insurance companies for services and allow them 45 days to render payment. After 60 days, you are responsible for the entire balance, paid in full. If you have any questions, our courteous staff is always available to answer any questions you may have.

New Patients

If you are new to the practice, please click on the documents provided in the Patient Forms section and have the appropriate forms filled out prior to your appointment. We ask that you arrive 10 minutes prior to your appointed time so that we can enter all of your information into our computers.

If you have dental insurance, please bring your ID card or a claim form that has the insurance company name, claims address and phone number as well as a group number.
